
D4 Medical
A D4 driving medical is a specific type of medical examination that is required for individuals who are applying for or renewing certain categories of driving licenses.
It is a standardised assessment that ensures that drivers meet the medical standards necessary for safe and responsible driving.
D4 driving medicals are required for operating lots of types of vehicles, and the assessment is regulated by the Driver and Vehicle Licensing Agency (DVLA).

HGV / LGV Medical Heavy Goods Vehicle / Large Goods vehicle
A medical to enable the driver to drive over 7.5t and up to 32 tonnes. A Full category C driving licence is needed, and the driver must meet DVLA Group 2 medical standards form (D4)

PSV / PCV Public Services Vehicle / Public Carrying Vehicle
A medical to enable the driver to drive public vehicles. The driver must have held a Full category B driving licence held for 2 years to operate these vehicles and must meet DVLA Group 2 medical standards form (D4).

D4 Medical
All operators of vehicles which are above 3.5 tonnes are required to pass a driving medical before undertaking their role. This includes motorhomes, forklift trucks and cranes over this weight.
All operators of vehicles within this section will require D4 medical before first operating one of these vehicles. After the age of 45 drivers are required to have a medical every 5 years up until the age of 65. After the age of 65 they are required to have a medical every year to enable them to continue driving vehicles over 3.5tonnes.
The Examination
During the D4 medical examination, which is conducted by a GMC registered Doctor (GP) they will assess various aspects of the driver’s health to determine their fitness to drive including their eyes with an eye test.
The D4 medical appointment includes the following components:

Registration
- A D4 form will be provided if the driver has not already received one, and health questionnaire to complete.
- A picture ID such as passport or driving licence will be checked by a member of the team.
Medical History
- GP will enquire about the driver’s medical history, including any pre-existing conditions, surgeries, medications, or disabilities.
- Health questionnaire will form part of the appointment.
Physical Examination
- Blood pressure, Weight (BMI), Height, Pulse and oxygen levels will be tested.
- Discussion with a doctor regarding any existing health issues and medication
- Listening to the drivers breathing and heart sounds with a stethoscope.
- External examination/ feel of the abdomen.
- Examination of the nervous system, looking at tone, power, reflexes, sensation, and coordination.
- Examination of the eyes and other nerves in the face
- Sight test- Distance only, with each eye done separately, with or without glasses.
- Balance Assessment and nerve test for the back of the brain.
- General movement of the back and neck.
Once the examination is complete, the GP will make a decision as to whether the driver has passed or whether they require additional tests before receiving a signed certificate, and their D4 form to send to the DVLA for approval. Any re-tests needed at our surgery are free to the driver.
These free re-tests may include the following
- Blood pressure reading at another date if the drivers blood pressure is too high.
- Completing the eye test again with a new glasses prescription.
- Checking blood sugar readings collected over a 2-week period at 2 hourly intervals when driving if diabetic.
If the driver has heart problems, then additional tests may be required such as an exercise tolerance test (ETT), or a stress echo (a specialist heart trace), and we can recommend an approved provider to carry this out.
If the driver has had a heart attack, or angina, then an echocardiogram (ECHO) may need to be carried out, which we can carry out at the clinic for £50.

Items needed for the appointment
- DVLA Standard D4 Medical Form if it has been posted as part of a license renewal (don’t worry if not, we can provide these at the clinic for you, but please allow extra time for completing)
- Photographic ID
- Glasses or contact lenses if worn for driving
- Any medication
It is advised to wear footwear which is easy to remove for the appointment.
